Is Cooper Center for Metabolism a good fit for me?
The following information should help you make a decision about whether CCM is a good fit for you. We are not a primary care facility. We specialize in obesity, diabetes prevention, prediabetes, metabolic syndrome (blood pressure, cholesterol, pre-diabetes, fatty liver), low blood sugar reactions, and metabolic causes of infertility. We work with patients of all ages and see many families who are affected since these conditions have a genetic basis.
Dr. Cooper and Alisha Boxley, ARNP, collaborate closely and apply the same methodology to their clinical approach. We track our outcome data and strive for the best results possible for our patients. Our goal is to prevent heart attacks, strokes, and diabetes. We aim to diagnose and reverse prediabetes, hypertension, high cholesterol, and obesity in our patients.
If you are affected by the areas that we specialize in, we may be a good fit for your needs. Please note that we are not a participating provider on any insurance plan, and therefore your out-of-network benefits are important to consider. We do not bill insurance and follow a subscription model. Appointments are usually spaced 2-3 months apart, depending on progress points and level of complexity.
We have formally opted out of Medicare, which means that while we can see you if you have Medicare coverage, you are not eligible for reimbursement from Medicare for services rendered by us.
What is the process for becoming a CCM Patient?
Beginning June 1, 2026, all new patients at Cooper Center for Metabolism are seen under a monthly membership. Membership is what gives you access to our team, services, and subscriber-discounted rates. Here is how to get started:
Fill out and submit the New Patient Inquiry Form below. This is a required first step — it gives us the information we need to assess fit and prepare for your scheduling call. You will be redirected to a calendar to select a time for your scheduling call after submitting.
Step 2 — Scheduling Call
A new patient coordinator will review your submission before your call. During the call, we will confirm your appointment date, answer questions about the practice, and walk you through the membership enrollment process. Note: scheduling coordinators cannot answer clinical questions — those are addressed at your first appointment.
Step 3 — Enroll in Full Membership
After your scheduling call, your coordinator will send you an enrollment link for Full Membership through our secure Hint Health patient portal. Complete the enrollment and add a credit card, debit card, or bank account (ACH) on file. Monthly membership begins on your enrollment date.
Step 4 — Establishment Visit
Your first appointment is your establishment visit, which includes an initial provider consultation and an in-clinic lab draw. A one-time establishment fee is applied at this visit ($630 Dr. Cooper / $545 ARNP), charged to your card on file.
Step 5 — Activate Your Hello Health Patient Portal
All patients use Hello Health PortalConnect as our secure communication platform. You will set up your portal account before or shortly after your first visit. There is an annual portal fee of $69 (individual) or $99 (family), paid directly to Hello Health — this is separate from your SPM membership.
Membership — What to Expect
Full Membership at Cooper Center for Metabolism is a month-to-month subscription with no annual contract. You choose your provider (Dr. Cooper or Alisha Boxley, ARNP) and your visit structure (Plan A or Plan B) at enrollment.
Plan A — Provider-Focused Care
4 clinical provider visits per year. Best for patients with active medication titration, new diagnoses, or frequent monitoring needs.
Plan B — Integrated Care Team
3 clinical provider visits per year plus 2 flex visits. Flex visits can be used for nutrition consultations, exercise physiology consultations, or a combination (2 nutrition, 2 exercise, or 1 of each). Best for patients seeking broader lifestyle and behavioral support alongside clinical care.
Both plans are the same monthly price. Both include all membership benefits: care coordination, prescription management, prior authorization, secure portal messaging, and a 10% member discount on all à la carte services.

Essential Membership
A Note on Essential Membership
Essential Membership is a reduced-frequency tier for established patients who are clinically stable and only need 2 provider visits per year. It is not a reduced-rate option — eligibility is based on clinical stability, not cost preference.
Eligibility is determined by your provider through a clinical chart review, not by self-selection. Patients must have been active with the practice for 3+ years to be considered. If you are a long-standing patient and would like to know if you qualify, please contact our office.

Optional Service Packages (Members Only)
Members Bundle and Save on Services
Prepaid service packages offer 25% savings off standard rates. Purchase a package anytime and use the sessions over the next 12 months. Packages are available for DEXA body composition scans, in-clinic lab draws, nutrition consultations, exercise physiology consultations, VO2 Max testing, submaximal exercise testing, and resting metabolic rate (RMR) testing.

We accept payment via bank transfer (ACH), debit card, credit card, and HSA/FSA card.
Credit card transactions include a card processing surcharge. ACH, debit, and HSA/FSA payments are not surcharged.
