Becoming a Cooper Center for Metabolism Patient
Is CCM a good fit for me?
The following information should help you make a decision about whether CCM is a good fit for you. We are not a primary care facility. We specialize in obesity, diabetes prevention, prediabetes, metabolic syndrome (blood pressure, cholesterol, pre-diabetes, fatty liver), low blood sugar reactions, and metabolic causes of infertility. We work with patients of all ages and see many families who are affected since these conditions have a genetic basis.
Dr. Cooper sees children under age 17 and has trained 2 additional providers who can accommodate new adult patients. All providers collaborate closely and apply the same methodology to their clinical approach. We track our outcome data and strive for the best results possible for our patients. Our goal is to prevent heart attacks, strokes, and diabetes. We aim to diagnose and reverse prediabetes, hypertension, high cholesterol, and obesity in our patients.
If you are affected by the areas that we specialize in, we may be a good fit for your needs. Please note that we are not a participating provider on any insurance plan, and therefore your out-of-network benefits are important to consider. We do not bill insurance and require payment at the time of service. We do provide you with superbills to submit to your insurance. The general appointment costs with medical providers range from $380-$490. Appointments are usually spaced 2-4 months apart depending on progress points and level of complexity.
We have formally opted out of Medicare, which means that while we can see you if you have Medicare coverage, you are not eligible for reimbursement from Medicare for services rendered by us.
What is the process for becoming a CCM Patient?
The Cooper Center for Metabolism (CCM) is a clinical facility dedicated to the diagnosis and treatment of underlying metabolic causes of type 2 diabetes, obesity, and infertility. In order to better serve you as a patient there are a few steps that we request potential patients to complete in order to assess if our services are the best fit for your needs.
This form is a required first step. It will ask you to acknowledge that you are aware of our pricing, clinic protocols, etc. It will also ask for important patient information that will apply to your scheduling call. This helps us create your new patient profile.
Step 2: After completing and submitting this scheduling form, you will be redirected to a calendar page that will instruct you to select an available date and time to set up a scheduling call with one of our new patient coordinators. Prior to your scheduling call, our provider team will review your submission and determine if you are a good candidate for our services. You will be notified via email if there is any indication that we may not be a good fit for you. During this call, our new patient coordinators will not be able to answer any medical questions. This call is strictly for scheduling purposes.
Step 3: Please have your preferred payment card ready during this call so we can enter it into your profile. We require that patients keep a preferred card payment on file before their first appointment date.
Step 4: Activate your patient portal in preparation to consult with your provider. Once you have been scheduled as a new patient, you will need to set up your patient portal! The portal provides a secure mode of communication that meets privacy standards, with the functionality required to provide the most effective healthcare. This portal subscription is required for all patients as we will use this for all communication as well as important patient updates. There will be an annual subscription fee of $69, which will be charged to your card on file.
Covid Policy Updates
As of April 3, 2023, we will no longer require proof of Covid vaccination for entry to the clinic. We continue to strongly encourage all of our patients to receive regular Covid boosters.
Other requirements remain in place:
- Wear an N95 mask while you are in the clinic (we can provide you with one if you do not have one already).
- We will continue our normal prior protocols related to exposures and symptoms as usual (please see our Covid page on the menu above).
Office Location & Directions
We are located on the north side of Magnolia, just past Fisherman’s Terminal. Street parking is available on W. Commodore Way, or turn right at the building and enter the parking lot behind the building. Park in any space designated as Suite 100, or a visitor space if the CCM/SPM spaces are full and you will be visiting for less than 2 hours. The spaces may have signs saying ‘Permit Parking Only’ but you may still park there.
